I know Mae got once costume deduction cause one of her feathers on her costume fell on the ice.

Do you remember some other instances?

I remember one instance with Wang/Wang, when Lei had to basically rip his tie off and throw it to the side - sadly I don't remember when this was exactly, nor am I able to find the video.

Poor Artur Danielian lost his necklace on the ice in his FP on a JGP last season, which likely threw off his concentration enough to make him flub his skate and miss his opportunity for the JGPF :(

Some of the judges deducted from her presentation mark for her costume. The mesh covered front panel of her purple dress showed quite a bit too much of her boobs for their liking.

There are currently two types of deductions related to costumes

The "costume failure" deduction, which is applied by the Referee, takes place when something from the costume falls on ice (Rule 501, Part II). The first three examples given in this thread are of this type.

Then there is the "Costume/prop violation" deduction for costumes that violate Rule 501, part I (At ISU Championships, the Olympic Winter Games and International Competitions, the clothing of the Competitors must be modest, dignified and appropriate for athletic competition – not garish or theatrical in design. Clothing may, however, reflect the character of the music chosen. The clothing must not give the effect of excessive nudity inappropriate for the discipline. Men must wear full length trousers. In addition, in Ice Dance, Ladies must wear a skirt. Accessories and props are not permitted. For Ice Dance, exceptions to the restrictions may be announced by the IDTC if appropriate to the Rhythms selected for the Rhythm Dance).
This is applied if a majority of the panel (judges + referee) calls for it.

On the latter, I remember Lorenz/Polizoakis got it at Tallinn Trophy 2017 in the SD (it was Lorenz's costume that was problematic), called by 6 judges out of 8. Then at Golden Spin, 3 judges out of 8 called it and so they avoided the deduction. However, not to risk, they slightly changed the dress for Olympics.

Interesting that it says "At ISU Championships, the Olympic Winter Games and International Competitions" but like the example I gave, are there any regulations for domestic competition?

I guess it says so because ISU regulate international events. National Federations decide on how domestic competitions are conducted. Many of them, especially the major ones like Nationals, are conducted under the International Judging System (IJS) and therefore the same rules of ISU international events are applied.

I wonder if that applied under 6.0 or if it would just be a case of the national federation following their own similar sort of guidelines.

I seem to recall it was a particular Katerina Witt costume (an almost none existent skirt) that was the cause of some of these regulations. I don't know if any of the judges marked her down for the offending costume or not at the time.

Among the costume failures I remember Elena Ilynikh lost one of her bracelet during the FD at 2017 Russian Nationals; they got a deduction and it costed them the third place (to Katsalapov/Sinitsina no less!) and the spot at world!

Under the 6.0 there was some kind of deducations for the costumes. I don't remember the details. One infamous episode was at 1989 European Championships when the British judge marked down Alexander Fadeev down because she could see his dance belt through his white trousers. She tries to explain it at minute 3:38: https://www.youtube.com/watch?v=D8ECupWBess

One time a costume problem actually helped a skater. Todd Eldredge fell on a triple Axel and lost a cuff link. He spotted it on the ice and stopped the program to retrieve it. The referee allowed him to restart the program from before the Axel attempt. (They couldn't be sure if the cuff link came off because of the botched attempt, or if was already on the ice and maybe caused the fall.)

On the second try he landed the jump and went on to win the competition.

At 2006 US Nationals, Emily Hughes lost an earring on the FS warmup and a search ensued. Luckily the earring was found by Emily herself and things returned to normal.

I think Belbin & Agosto ? had a costume mishap where a decoration on her skirt came off and the commentators kept commenting on it every time they got near it. I can't recall the event. I think the dress was red or pink. The problem didn't affect their placement.
