It's unusual to see a dance team do more than stumble in a routine. In 2008 at Cup of Russia, Charlie White stumbled once and fell twice in an OD, dropping the team of Davis & White to eighth in that segment of the competition.
That's probably the biggest dance meltdown I recall.
In pairs, it's the other way--perfect performances are rare. Meltdowns are quite common.
