I think this emoticon references an old comedy bit in movies, tv sitcoms, sketches, etc. If one side in a battle wanted to surrender but didn't have the standard white flag, they would tie their white undershorts to a stick and wave that. The joke is "We're so beat down, desperate, and anxious to surrender that we couldn't even get it together to find the white flag. We just grabbed our underwear and ran that up."
