I believe the host federation decides and yes skaters can choose to not participate if they are injured or unwell. If they simply just dont want to, well....it would make you look terrible and rude.

I remember reading that they'd be fined if they just didn't show up, like Yulia was fined for not showing up to the medal ceremony

It has not been decided yet. They will decide it in the spring or at the beginning of summer. And I think there were 4 more girls in this performance than in competitions.

Re synchro and the Olympics: per a Hersh article from Sep 2014 in an old GS thread, the IOC decision will be known by April.

The request is in the hands of the IOC’s executive board, which will approve the final 2018 program and athlete quotas next April.

I think if they refuse they have to pay for their accommodations. (And airfare? Does the LOC for Worlds pay for flights? I know the GP does.)

I believe they have to pay a fine if they are supposed to be in the gala & pull out without legitimate reason...there was some discussion about this when Florent pulled out of the TEB gala.
