The ONLY time I ever saw that happen (by tv at least) was in Sochi when Plushenko w/d.

Bull. Saw it happen in Boston when the crowd filed out en masse after Hanyu's SP with Kovtun still to skate.

ETA: Correction: it's the only time I remember it happening, when I was watching. I do have a tendency to forget bad things sometimes.
